I understand and agree to the following:
Pick-up Tardiness Policy / Late Payments / Returned Checks:
Preschool hours are until 12:30pm for our Pre-K students and until 5:30 p.m. for our School-Age students. In the case of tardiness, I acknowledge by my signature below that I will be charged a late fee of $15 per 15 minutes to begin at 12:31pm or 5:31pm. I also acknowledge that there will be a $30 fee for all returned checks. I understand that payment is due for registered weeks regardless of my child's attendance and if payment is consecutively late 2 weeks in a row The Learning Treehouse will pause your child's enrollment until tuition is caught up. In which case, tuition will continue to be due during paused enrollment to hold your child's spot in our program.
The Learning Treehouse Program Agreement:
I, the undersigned, as the parent or legal guardian of the child identified above, hereby give my full consent and approval for my child to participate in activities at The Learning Treehouse, located at 3928 N Croatan Hwy, Kitty Hawk, NC 27949. I understand that there are certain risks of play that may result in damages and injuries in the participation of my child during these activities and I hereby accept these risks on behalf of my child.
I hereby certify that my child is fully capable of participating in gym and playground/area activities and that my child is healthy and has no unknown/hidden physical or mental disabilities or infirmities that would restrict his/her full participation in such activities.
I hereby, for myself and on behalf of my child, agree to save and hold harmless and fully indemnify The Learning Treehouse Management and Staff, on the account that all normal safety procedures have been taken on behalf of Management and Staff, from any and all liability for any personal injury or injury to any third-party child resulting from my child's actions/participation in the above-mentioned activities.
I hereby release The Learning Treehouse and its entire staff from any and all liability, on the account that all normal safety procedures have been taken on behalf of Management and Staff, for any injuries that my child may sustain as a result of any activities taken place at the above-mentioned locations during the designated daycare hours for the duration of my child's time at The Learning Treehouse. I understand that my child will be physically active including participating in movement exercises.
I hereby authorize The Learning Treehouse LLC to publish the photographs or video taken of me or my child, for use in The Learning Treehouse printed publications, on-line media outlets and website. I hereby agree to irrevocably release and waive any claims that I have now or may have hereafter against The Learning Treehouse LLC, and its instructors.
I have read and fully understand and agree to the above terms of this Liability Waiver Agreement, Medical Policies, and Pick-Up Polices. I am signing this agreement voluntarily and recognize that my signature serves as complete and unconditional release of all liability to the greatest extent allowed by law in the State of North Carolina.
